Cigna and Aetna. 

 

If your insurance provider is not listed A.C.T.S. would be considered an out-of-network provider.  If your insurance company allows, you will be able to submit an insurance claim for reimbursement and A.C.T.S. will furnish you with an invoice containing all of the required information needed for submission.

Payment Forms

We accept cash (cashapp & zelle) and debit/credit cards. All co-pays, deductible amounts, and private pay fees are due at the beginning of each appointment, unless other arrangements have been made in advance.

Missed Appointments

 

If there is an missed appointment or if a 24 hours advance notice of cancellation of an appointment is not provided there is a $65 charged for the missed appointment or late cancellation.

Paperwork Requests

 

​Any letters, forms, or any other paperwork to be completed, such as FMLA or disability forms, please be advised that there is a fee for paperwork. The fee is $150.00 per hour. FMLA paperwork generally requires a minimum of 30 minutes to complete, due to the need for supporting clinical documentation. Short-term disability often takes longer to complete, and may require additional assessments beyond the regular intake evaluation. The time required to make copies or prepare and send faxes, and any other administrative business (e.g. preparing releases of information or requests for records; communication to lawyers or other non-clinical calls or emails) not directly related to the provision of clinical services, will also be assessed based on a rate of $150.00, with a minimum fee of $25.00.

FMLA, disability, other paperwork or letters of support will only be made after having at least 6-8 sessions.  FMLA or disability paperwork will only be completed if the clinician believes they can support it's necessity based on what has been presented at intake and during sessions.
